Estimation of cardiac amyloid infiltration with myocardial T1 mapping correlates with severity of cardiac involvement

چکیده
Background Post-contrast myocardial T1-mapping with cardiac magnetic resonance (CMR) has shown usefulness in the diagnosis of cardiac amyloid. A lower value indicates increase in extracellular volume and thereby amyloid deposition. The aim of this study is to assess the association between the amount of amyloid deposition as estimated by the degree of post-contrast myocardial T1 reduction and the severity of left ventricular (LV) involvement in cardiac amyloidosis.

T1 mapping shows increased extracellular matrix size in the myocardium due to amyloid depositions.
Amyloidosis is a systemic infiltrative disorder in which insoluble protein fibrils are deposited in the extracellular matrix (ECM). The prognosis is predominantly determined by cardiac involvement because the amyloid depositions lead to a restrictive cardiomyopathy. Although endomyocardial biopsy is the gold standard for diagnosing cardiac amyloidosis, the associated risk for complications favo...
متن کاملCardiac amyloid burden assessment by T1 mapping predicts survival in patients with systemic AL amyloidosis - a 2 year follow-up study
Background Cardiac involvement drives outcome in systemic AL amyloidosis. Late gadolinium enhancement (LGE) CMR is useful for the detection of cardiac amyloid, but characteristic LGE patterns do not always occur or may appear late in the disease. Using CMR T1 mapping, we measured the pre contrast myocardial T1 and myocardial Extracellular Volume (ECV), reflecting myocardial amyloidosis burden a...

BACKGROUND Cardiac involvement is common in amyloidosis and associated with a variably adverse outcome. We have previously shown that cardiovascular magnetic resonance (CMR) can assess deposition of amyloid protein in the myocardial interstitium. In this study we assessed the prognostic value of late gadolinium enhancement (LGE) and gadolinium kinetics in cardiac amyloidosis in a prospective lo...

Purpose: To assess the relationship between the severity of coronary arteries involvement and the extent and pattern of myocardial scars in Cardiac Magnetic Resonance of patients with history of remote myocardial infarction. Materials and Methods: The Cardiac Magnetic Resonance images of sixty patients with history of remote ST segment or non-ST segment elevation myocardial infarction were ...
متن کاملEarly post-contrast T1 mapping yields maximal discriminatory capacity for detection of cardiac amyloid - influence of temporal T1 differences on MOLLI imaging
Methods Myocardial T1 mapping was performed in two cohorts (1) “amyloid +” subjects, defined by biopsy-proven systemic amyloid with associated remodeling suggestive of cardiac involvement (left ventricular [LV] hypertrophy and/or atrial dilation); (2) normative controls without risk factors for amyloid or cardiovascular disease. CMR (1.5T) included 2 components cine-CMR (SSFP) for cardiac struc...
